Another tough one. There was one particular moment that sprang to mind (spoilers for FFVII), but I'm saving that one for another category if any of the listed categories turn out to be unimaginative (which looks to be likely). In fact, you could probably throw a dart at a list of FF* plot points and come up with a suitable WTF moment - either in the sense of "woah, plot twist!", or in the sense of "oh Japan, you so crazy" (yep, we're all thinking of this)
[Obviously, spoiler alerts. Highlight any greyed-out text to reveal it.]
As gaming has matured as a genre, and developers have become more aware of story structure and pacing, there have been some excellent twists and reveals. Some of them, like the ending of the first act of CoD4 or the finale of Half-Life 2, come completely out of the blue and contravene your basic assumptions about the nature of game protagonists (namely, that they're immortal and control their own destiny)
Sucks to be you, buddy boy...
Well that was unexpected (you know, if you haven't played the first game...)
Some of them, like the reveal of Star Wars: Knights Of The Old Republic (Throughout the game, you have been acting in the aftermath of great death and destruction caused by a powerful Sith Lord. Partway through, it is revealed that you are that Sith, who was brain-wiped by the Jedi into becoming an ally and forgetting his past), or the ending of Metroid (worst kept secret in gaming history: Samus, the bad-ass, monster-hunting, galaxy-trotting bounty hunter you've been playing as all game, reveals his helmet to reveal that he is in fact a girl), throw the whole game into a completely different light (and in that last case, arguably make big strides in widening gaming's demographic).
And some of them just make you go "WTF!?" with frustration...and the greatest of all of those, the one most deeply ingrained into the collective gaming psyche and the cause of broken controllers the world over, is this:
Frickin' Toad...
